Adult Membership Agreement

In applying for a new or renewal membership in the Indianapolis Ski Club, Inc., I recognize the Club as a volunteer non-profit organization, organized solely for the benefits of its members.

I acknowledge that hazards are inherent in the sport of skiing and other activities participated in by Club members and their guests and hereby assume all risks of injuries or damages incidental to such activities. Until I notify the Indianapolis Ski Club in writing of my prospective revocation of this release, in consideration of my new or renewal membership, I will abide by the Articles of Incorporation, by-laws and policies and do, on my own behalf and that of my heirs, executors, administrators, and assigns, release and forever discharge the Indianapolis Ski Club, its officers, directors and members from any claims, demands, damages, actions or causes of action that could hereafter arise from my participation in Club-sponsored trips or other activities.
I have read the above agreement and release, and by checking this box, I authorize this as an electronic submittal of my signature and the signature of my other family members added through this application.
